FORMS IN CAMPERS ONLINE ACCOUNT

 

To better serve you, several camper forms are now ONLY available through your camper's account and must be completed online in order to attend camp.

 

Please review each page in your camper's account to avoid missing a signature or required information which can delay the process.

FORMS TO BRING TO CAMP CHECK-IN

 

Please fill out these doctor signature forms*

and bring with you to Opening Day!  

This will prevent them from being lost by fax or email.

 

IMPORTANT NOTE:  

All online and doctor signature forms* are required in accordance with state and American Camping Association guidelines before a camper can attend camp.  

  **Camper medication, supplements, and vitamins will need to be turned in to the RN/NP/MD upon arrival. 
Please bring the original bottle or box along with doctor's orders for each item. 
If you have a copy of the doctor's orders from your physician's office or school already available,
you are welcome to bring that instead of the Doctor's Order Form above.

On Opening Day, additional instructions can be given to the RN/NP/MD
on how and when to administer these items.

*IMPORTANT NOTE ON CABIN MATE/BUDDY REQUESTS*

CAMP POLICY: Campers may request ONE roommate (overnight camp) or buddy (day camp) per session. Only RECIPROCAL requests between

two campers will be honored. The requested roommate or buddy must also be within 12 months of the camper's birth date. 

Campers are placed in cabins (overnight camp) or small groups (day camp) based upon gender, age, and grade. Within these parameters,

we strive to honor ONE request per camper but cannot always guarantee their placement.  All groups are balanced between

campers with roommate or buddy requests and those without. All campers will be grouped within the same 12-month age group

pending an unusual breakdown of age ranges in camper registrations.

 

If you would like to find out if we have your child’s request logged in our system,

please contact office@camptimberline.com one week prior to attending camp.

 

Roommate or Buddy Request Instructions:

If your child would like to request a roommate or buddy to be in the same group with them,

please be sure to enter the following on your camper’s account:

  1. Same first and last name of the other camper

  2. The email address the other parent/guardian used to register.

This will provide a DIRECT MATCH in our system.

Example: Sarah Anderson (age 10 years, 2 months) requests Brooklyn Smith (age 10 years, 7 months) and Brooklyn Smith requests Sarah Anderson.
