Please fill out these health forms

and bring with you to Opening Day!  

This will prevent them from being lost by fax or email.

 

IMPORTANT NOTE:  

All online and printed health forms are required in accordance with state and American Camping Association guidelines before a camper can attend camp.  

 **Camper medication, supplements, and vitamins will need to be turned in to the RN/NP/MD upon arrival. 
Please bring the original bottle or box along with doctor's orders for each item. 


On Opening Day, additional instructions can be given to the RN/NP/MD
on how and when to administer these items.

We can accept versions/copies of the Doctors Order or Emergency Care Plans
already generated b your school or physician's office to make it easier as well!

FORMS IN CAMPERS ONLINE ACCOUNT

 

To better serve you, several camper forms are now ONLY available through your camper's account and must be completed online in order to attend camp.

 

Please review each page in your camper's account to avoid missing a signature or required information which can delay the process.

ROOMMATE/BUDDY REQUESTS

Camp Timberline offers Roommate Requests (overnight camp) and Buddy Requests (day camp) so your child has the option to attend camp with a friend. This is not a requirement for a fun week at camp and our ultimate goal is for each small group to have a positive and connected time at camp with their peers and counselors.

To accomplish this goal, campers are divided into boy and girl small groups and then placed by age and grade.

Within these parameters, we strive to honor ONE request per camper but cannot always guarantee their placement. All campers will be grouped within the same 12-month age group pending an unusual breakdown of age ranges in camper registrations.

Since this system has many variables, we ask parents/guardians to partner with us in clearly selecting a Roommate or Buddy Request with the guidelines below and to be flexible with each camp session's dynamics and potential.
